A: Placed IM pin ESF tie in to repair non-healing proximal ulna fracture. Had difficulty with repair due to distal fracture already healing.
P: Change wing wrap tom to check surgery site.

Procedure - Fracture repair - ulna | Performed by - AD | Surgery time - 75 minutes | Pre-medicated with torb IM, anesthestized via isoflurane facemask,
then intubated with 3.0 uncuffed ET tube. Placed in sternal
recumbency, plucked and scrubbed dorsal aspect of ulna. Incised
over the proximal fracture site with #15 blade, bluntly dissected
down to bone. Removed small amount of fibrous callus, no bony
callus present. Placed IM pin in the proximal fragment and drove
normograde across the fracture to seat in the distal fragment.
Tried to drive pin up the distal fragment but pin came out the
dorsal aspect of the midshaft fracture site where the callus
was softest. Struggled to get pin to drive up the distal aspect
of the bone as unable to really manipulate or move partially
healed bones that were in slight malalignment. Incised over
the fracture site, cleaned out old blood and flushed area with
saline and with visualization was able to get the pin to properly
go up the distal fragment to seat at the carpus. Closed incision
sites with 4-0 webcryl in simple continuous patterns. Placed
2 cross pins, one in each fragment, bent cross pins and IM pin
around the dorsal aspect of the wing and secured together with
epoxy. Covered sutures with TAO, telfa/tegaderm, placed fabric
wrap on wing. Patient recovered uneventfully from anesthesia. |

O: Noted 1 molted R primary feather in cage. Removed L figure 8 wrap. L ulna fracture still unstable, but alignment still feels decent. Performed very gentle PT to wrist and elbow. Was not able to get accurate PROM measurements w/ unstable fracture. Applied L figure 8 wrap and covered with bandage tape. LD3 scabbing static and healing well. No flukes in mouth noted. Bird stoic in hand. Gave oral meds. No leftovers.

Alignment okay but may benefit from surgical repair Initial treatment: 0.14cc Torb IM, 0.11cc Simbadol IM, 30cc LRS SQ in right inguinal area, 0.23cc Meloxciam PO, 1.03cc Calcium Slurry PO. Placed left wing figure 8 wrap.
A: Likely hit by car as found in center of the road. L ulna fracture in okay alignment and radius intact. Possible that conservative management in a wrap will be sufficient for healing but may benefit from surgical pinning. Left eye multiple retinal tears near pectin but seemed visually reactive OU.
P: Rx Tramadol, Meloxicam, Calcium Slurry, fluids. Change wrap in 3 days, repeat rads next week and decide if surgery is necessary. Collect blood and fecal tomorrow.
